[b]Name:[/b] The new name the Undead goes by - may be the original human or Guardian Beast name, too.
[b]Former Human Name:[/b] The name the original human had
[b]Former Guardian Beast Name:[/b] Self-explainatory, the name if the Guardian Beast that fused with the human soul.
[b]Element:[/b] Every Guardian Beast has an inherted element, be it a classic element like fire or water, or an exotic one like poison or Reishi. This determines the basic outlines of your power.
[b]Age:[/b] Age they died at, and current age. Remember that you can't be older than 'relatively' humanity (around 20000 years MAX!)
[b]Gender:[/b] Male, Female, Trap, Reverse Trap? Some Undead can actually shift genders in their release-forms, if your character can do that, put the weakest release forms gender here.
[b]Orientation:[/b] Neutral, Evil, Goodfag? This tag is optional.
[b]Appearance:[/b] Two Paragraphs minimum. Pics can be included or used instead.

[b]Personality:[/b] Minimum is 2 Paragraphs, the longer the better.

[b]Death/Awakening:[/b] Their life till their death, their awakening, finding of the real body, destruction of it, and further history.

[b]Human Powers:[/b] The powers in human form. Remember, those are usually weaker than a Shinigami in Shikai. Only Jokers can get to a Shikai-like level as humans.
[b]Weapon:[/b] Undeads are able to create weapons from their own selves or use technology and artifacts. You can chose not to use a weapon in human form. Weapons in other forms are listed as powers.

[b]Monster Form:[/b] The appearance of this monstrous form, and it's natural weapons.
[b]Monster Powers:[/b] The power of the monstrous form is slightly higher than a normal Shikai, Jokers reach almost Bankai level in this.

[b]Guardian Beast Form:[/b] The appearance of this wondrous form. Appearance is most likely a bit animalistic still, but mostly humanoid (think catgirls for a rough example). There are some examples to this, where the Guardian Beast Form looks more like an 'evolved' or 'surpassing' version of the Monster Form.
[b]Guardian Powers:[/b] Think Bankai, and you know the strength of this stage. Jokers surpass a normal Bankai usually.


[b]RP Sample:[/b] The better this is, the higher the chance for a high tier!
